A graduate of the prestigious Kasr Al-Ainy Faculty of Medicine at Cairo University, Prof. Hossam Salah embarked on a career filled with achievements. His journey was not limited to local excellence but extended to obtaining the highest international qualifications, most notably the **Fellowship of the Royal College of Surgeons of England (FRCS)**, a certificate reflecting the highest level of surgical competence.

He held the position of Professor of Orthopedic and Spine Surgery at Cairo University and led the Spine Unit at Nasser Institute as its director, the largest specialized center in Egypt. His expertise extended to working as a visiting surgeon in Libya and the UAE, where he founded the first center for spinal deformity surgeries in Libya.

Recurrent Disc Herniation

The statement "A herniated disc will definitely recur after surgery" is incorrect. Recurrence of disc herniation happens in only 3 to 5% of cases. In other words, 95-97% of patients do not experience a recurrence. It happens because we only remove the part of the disc pressing on the nerve, leaving the rest to function. A recurrence means a new part of the disc has moved to press on the nerve.

Vertebral Stents

Similar to cardiac stents, we now use stents in the vertebrae. If a vertebra is fractured or compressed due to osteoporosis or a tumor, we can insert a stent instead of performing fusion surgery. The stent is inserted through a 3mm incision while collapsed and is then expanded inside the vertebra with special tools to support it and restore its natural shape. It's a minimally invasive procedure with small incisions.

Cervical Disc Herniation

This causes neck and arm pain with numbness. These symptoms occur when part of the disc moves out of place, compressing the nerves, and sometimes the spinal cord, causing imbalance. If there is only pain, treatment begins with medication and physical therapy. If this fails or if there is muscle weakness, surgery is the solution.

Percutaneous Fixation

With technological advancements, there's been a focus on performing surgeries through small incisions, known as minimally invasive surgery, to reduce pain and shorten recovery time. Example: Percutaneous spinal fixation. Instead of a 20 cm incision, a 5-day hospital stay, and a two-month recovery... now it's a 3 cm incision, discharge the next day, and a return to normal life in a few days.

Lumbar Disc Herniation & Back Pain

This is when the gel-like center of the disc protrudes, pressing on the nerves that supply the legs. Therefore, the main symptom is leg and thigh pain (sciatica). Symptoms usually start with brief lower back pain that subsides as leg pain appears. In conclusion: A herniated disc causes leg pain, and in most cases, without lower back pain.

Cortisone

"Cortisone is a harmful and dangerous drug." I hear this daily. The truth is, cortisone is a hormone in our bodies that we cannot live without. It is not a toxic substance. It's the most potent anti-inflammatory. Its bad reputation comes from side effects (high blood pressure, high blood sugar, osteoporosis) with long-term, continuous use. Local injections do not cause these issues. Conclusion: It is a vital medication that must be used under medical supervision.

Osteoporosis & Spine Pain

It's a common misconception that osteoporosis causes pain. It is a silent disease and only causes pain if fractures occur. Some of these are microfractures that require expertise to diagnose. Especially in the spine and sacrum. It can lead to microfractures, causing undiagnosed lower back pain. Diagnosis can lead to a cement injection (sacroplasty), which relieves the pain.

Scoliosis & Waiting

The advice to "wait until age 18 to have surgery" is a huge mistake. Scoliosis primarily affects girls around adolescence. Accurate and early diagnosis and early surgery are the best solutions for excellent results. Unnecessary waiting is a major error that increases the curve's severity and reduces the correction achieved by surgery.

Disc Herniation & Paralysis

The statement "If I don't have surgery, I'll be paralyzed" needs clarification. Disc herniation is very common, and 70% of cases are treated with medication and physical therapy. Only 30% require surgery. Less than 5% develop nerve damage and leg muscle weakness, requiring immediate surgery. But for the vast majority, there is no fear of paralysis, and treatment is successful without surgery.
